الفتق الإربي (الفتق المغبني) وطريقة علاجه

الدكتور نضال عرفات اخصائي جراحة عامة وجراحة الجهاز الهضمي يتحدث عن الفتق الإربي (الفتق المغبني) وطريقة علاجة.

يحدث الفتق لدى الإنسان نتيجة حدوث فتح أو ضعف في الغشاء الذي يبطن عضلات جدار أحد أجزاء الجسم، وعادة ما يحدث الفتق في منطقة البطن أو منطقة الفخذ أكثر من غيرهما من مناطق الجسم، ويظهر الفتق جلياً على الإنسان من خلال حدوث انتفاخ في المنطقة التي حدث فيها هذا الفتق، وإذا ما كان الفتق قد حدث في منطقة البطن فإنه يسمى في هذه الحالة بالفتق الأربي وتظهر الانتفاخات في هذه المنطقة بصورة كبيرة عند حدوث شد في عضلات البطن، ويحدث الفتق لعدة أسباب من أبرزها تلك التي تنتج نتيجة ممارسات الإنسان أو عمله والتي تحدث ضغطاً على منطقة البطن، إلا أن الوقاية من الإصابة بالفتق أمر في غاية السهولة حيث يتحقق عن طريق الوقاية والابتعاد عن الأسباب التي أدت إلى حدوث هذا الفتق، وسنعرض في هذا المقال المعلومات حول طرق الوقاية من الفتق الأربي.

طرق الوقاية من الفتق الأربي
1. إذا كان سبب حدوث الفتق الإربي هو السعال المزمن، فإن الوقاية تكون عن طريق تناول الأدوية التي تعالج هذا السعال.
2. إذا كان سبب حدوث الفتق الإربي هو الإمساك المزمن، فإن الوقاية تكون عن طريق تناول الأدوية التي تعالج هذا الإمساك، والإكثار من الأطعمة الغنية بالألياف.
3. ينصح بالابتعاد عن حمل ورفع الأوزان الثقيلة إذا ما كانت هي السبب وراء حدوث الفتق.
4. يجب الحرص على ممارسة التمارين الرياضية التي تلعب دوراً في تقوية عضلات البطن، إلا أنه يجب الابتعاد عن تلك التمارين التي تعتمد على حمل الأوزان الثقيلة.

علاج الفتق الأربي
1. غالباً ما يتم علاج الفتق الإربي عن طريق الخضوع لعملية جراحية على يد طبيب مختص.
2. في بداية العملية وقبل إجراؤها فإنه يتم القيام بتعقيم المنطقة التي يوجد بها الفتق ومن ثم تخديرها.
3. يكون الهدف من إجراء هذه العملية هو القيام بعزل الأنسجة الطبيعية عن الأنسجة التي تعاني من الخلل والموجودة في منطقة الفتق.
4. يتم القيام بفتح الأنسجة وإجراء العزل ومن ثم القيام بإصلاح الأنسجة التي تعاني من وجود خلل.
5. بعد الانتهاء من إجراء هذه العملية فإنه يتم القيام بإغلاق الفتق بطريقة الخياطة المناسبة لذلك.
6. يجب توخي الانتباه والدقة عن إجراء هذه العملية وذلك تجنباً لحدوث أي مضاعفات في الأنسجة المحيطة بالفتق.
7. قد يعاني المريض من حدوث ضعف في الأنسجة بعد إجراء العملية وهذا يسبب حدوث عدة فتوق أخرى.
8. يوجد العديد من التقنيات الحالية والحديثة والتي تجنب الإنسان تلك المضاعفات ومنها تقنية المناظير.

أنواع البواسير وطرق علاجها

الدكتور نضال عرفات اخصائي جراحة عامة وجراحة الجهاز الهضمي يتحدث عن أنواع البواسير وطريقة علاجها.

البواسير
البواسير هي عبارة عن كتل لحمية مملوءة بأوعية دموية منتفخة توجد في القناة الشرجية أسفل المستقيم، يصاب بها الرجال والنساء على حد سواء، وترتفع نسبة الإصابة بها كلما تقدم الإنسان بالعمر، كما أن النساء الحوامل أكثرَ عُرضةً للإصابة بها.

وتقسم البواسير الى نوعين وهما بواسير داخلية وبواسير خارجية، وتقسم إلى أربعة درجات عند تحولها إلى حالة مرضية، وهي كما يلي:

الدرجة الأولى كتلة لحمية دموية داخل قناة الشرج.
الدرجة الثانية كتلة لحمية دموية داخل الشرج وتخرج خارج فتحة الشرج عند الإخراج وتعود إلى داخلة لوحدها.
الدرجة الثالثة كتلة لحمية دموية داخل الشرج وتخرج خارج فتحة الشرج عند الإخراج ولا تعود لوحدها ممّا يضطر المريض إلى استخدام إصبعة لإدخالها مكانها.
الدرجة الرابعة هي عبارة عن بواسير داخلية خارجية تتدلى من فتحة الشرج، ولو حاول المريض إدخالها للداخل لن يتمكن من ذلك.

أسباب الإصابة بالبواسير
لا يوجد سبب محدّد يؤدّي للإصابة بالبواسير لكن هناك بعض العوامل التي تزيد من فرص الإصابة بالمرض مثل:

حالات الإمساك الشديدة و المتكرّرة.
تجمّع الدم في الأوردة الدمويّة الموجودة في منطقة الشرج.
انعدام النظافة الشخصيّة التي تسبّب تجمُّع البكتيريا.
السمنة المُفرطة.
الاعتماد على نظام غذائي خالي من الألياف.
الجلوس لفترات طويلة.
حدوث الحمل قد يسبّب الإصابة بمرض البواسير.
التقدم في العمر يسبّب ضعف في الأوعية الدمويّة وتمددها.

أعراض الإصابة بالبواسير
يوجد هناك الكثير من الأعراض التي تدل على إصابة الشخص بالبواسير وفيما يلي أبرزها:

نزول الدم الأحمر القاني من فتحة الشرج.
كتل متدلية من فتحة الشرج.
إفرازات مخاطية من فتحة الشرج.
حكة شديدة في فتحة الشرج.
ألم في فتحة الشرج عند الإخراج.

طرق علاج البواسير
يوجد طرق علاج مختلفة للبواسير، وذلك حسب درجتها، وفيما يلي أبرز تلك الطرق:

علاج البواسير من الدرجة الأولى والثانية
وتتمثل طريقة علاج البواسير من الدرجة الأولى والثانية من خلال العلاج الموضعي باستخدام المراهم والتحاميل والعلاج التحفظي والمتمثل بمجموعة من النصائح الطبية التي ينصح فيها المريض، وفيما يلي أهم تلك النصائح:

الإكثار من تناول الخضروات والفواكة.
تناول الكثير من السوائل.
ممارسة الرياضة لخسارة الوزن الزائد وتنشيط حركة الأمعاء وبالتالي يؤدّي انتظام الإخراج لدى المريض.
استبدال الخبز الأبيض بالخبز الأسمر.
تجنب الإمساك والمحافظة على نظافة المنطقة المصابة.

علاج البواسير من الدرجة الثالة والرابعة
أما في الدرجة الثالثة والرابعة يتطلب ذلك التدخل الجراحي للعلاج والتي يخشى المرضى القيام بها خوفاً من الألم، ولكن للتطوّر التكنولوجي ظهرت التقنيات التي ساهمت في علاج البواسير دونَ التدخّل الجراحي كالعلاج بالليزر.
